Even pianos that haven't been tuned for decades can be tuned, and restored to playable condition. I am committed to tuning pianos regardless of their age or past neglect. If today is the day you are ready to have your piano tuned, than I would be thrilled to give it my time and attention!
A piano that hasn't been tuned in a very long time will not stay in tune as long as a regularly tuned piano would. But, after a few sessions (preferably 3-6 months apart) the piano should begin to stabilise.
In rare circumstances, a piano is"untunable" because of severe structural damage to the pin block. While pianos naturally deteriorate over time (felt compresses, strings corrode, wood changes), regular maintenance will extend its lifespan considerably.
